Invite your safety lead to bring a redacted current form, mark required fields and proposed label changes, then compare the closest existing form with that sample during the demo. Ask the provider to confirm the scope, timing and terms of any changes. A required-field check does not prove an answer is correct; have your safety lead review the answers and wording against your approved procedure.
